Terms and Conditions
Citi’s My Unforgettable Story Raffle Promo (“Promo”) will run from February 26 to August 31, 2015 (“Promo Period”).

• The Promo is open to principal and supplementary Citi Cardholders (“Cardholder”):
• Whose credit cards (“Card”) have been locally issued by Citibank, N.A. Philippines (“Citi”), are active and in good credit standing and with available credit limit; and
• Who are not prohibited under applicable Gifts, Anti-Bribery and Corruption laws, regulations, and policies from participating in and/or qualifying for this Promo.
• Officers and employees of Citi, of its advertising and PR agencies, and of its third party service providers, and their relatives up to the second degree of consanguinity or affinity, are likewise not eligible for this Promo.

Earning of Raffle Entries:

The following are the conditions for earning of raffle entries for “Log-in to Citibank Online”, the Cardholder must log-in to Citibank Online; each log-in earns 2 raffle entries; provided that, he/she can earn a maximum of 10 raffle entries per month.

To qualify for entries for the “Register your mobile number”, the Cardholder must not have an existing mobile number in Citi’s records prior to the Promo, and he/she must update his/her mobile number within the Promo Period by calling +63 2 995 9999.
To qualify for entries for the “Register your email address”, the Cardholder must not have an existing email address in Citi’s records prior to the Promo, and he/she must update his/her email address within the Promo Period by calling +63 2 995 9999.
To qualify for entries for the “Update your mobile number”, the Cardholder must have an existing mobile number in Citi’s records prior to the Promo and he/she must update his/her mobile number within Promo Period by calling +63 2 995 9999. Only one change in mobile number will be qualified to earn raffle entries.
To qualify for entries for the “Update your email address”, the Cardholder must have an existing email address in Citi’s records prior to the Promo, and he/she must update his/her email address within the Promo Period by calling +63 2 995 9999. Only one change in email address will be qualified to earn raffle entries.
To qualify for entries for the “Apply for a Citi Card via www.citibank.com.ph”, the principal Cardholder must apply online at www.citibank.com.ph and his/her application must subsequently be approved during the Promo Period. The approved Card can be the Cardholder’s first Citi Card or an additional Card.
To qualify for entries for the “Apply for and activate your new Citi Card by using it for a purchase”:
The Cardholder must not have any existing locally issued Citi principal Card in the last six (6) months prior to the time of application and must apply for a Citi Card as principal Cardholder within the Promo Period;
The principal Card must be approved during the Promo Period; and
The Cardholder must make a purchase within 60 days from date of Card issuance.
To qualify for entries for the “Avail of a Citi Personal Loan”, the loan availment must be made within the Promo Period.
Every month, selected transactions will earn double raffle entries as follows:

• A Cardholder can double his/her total raffle entries by registering for the Promo online. The Cardholder can visit www.citibank.com.ph/mystory and input his/her birthday, last 4 digits of his/her Citi Card, and his/her registered mobile number. The registered mobile number is the Cardholder’s last updated mobile number on Citi’s records.
• The Cardholder only needs to register once.
• Principal and supplementary Cardholders need to register separately.
• Only valid registrations will qualify for the double raffle entries.
• The following transactions will not qualify for this Promo: existing Citi PayLite® Installment Plan amortizations, credit card fees and charges, transactions for business purposes or for non-personal purposes, and transactions that are subsequently cancelled or refunded.
  Citi will generate raffle entries electronically. The number of raffle entries that the principal Cardholder and each supplementary Cardholder earns will be counted separately. The eligibility of the supplementary Cardholder’s transaction to earn raffle entries is dependent on the principal Card’s credit standing.

Drawing of Winners:
The Raffle Draws will be held in the presence of DTI and Citi representatives at the 18th Floor Citibank Square, #1 Eastwood Avenue, Eastwood City, Barangay Bagumbayan, Quezon City. The Raffle Draw Dates, corresponding transaction periods, and number of winners are as follows:

A Cardholder may win only once during the entire Promo Period.
The Cardholders’ accounts must be active and in good credit standing at the time of the draw and until the claiming of the raffle prize. The principal Cardholder’s credit standing at the time of drawing of winners and claiming of prizes will affect the supplementary Cardholder’s chances of winning.
Cardholders who qualified for raffle entries on account of enrolment for e-Statement, creation of a Citibank Online account, enrollment for Citi Alerts, or application for a new supplementary Card/s, must maintain his/her enrollment in the facility, his/her Citibank Online account, and/or the supplementary Cards applied for until the time of the draw and the claiming of raffle prizes.
A Cardholder whose name is drawn will be disqualified in the event that his/her Citi Card account/s is/are not maintained in good credit standing; his/her Card account/s become/s delinquent or is/are cancelled, not re-issued or terminated during the Promo Period; and/or his/her new Cards e-Statement enrolment, new Citibank Online account, new Citi Alerts enrollment, or new supplementary cards are no longer active at the time of the draw and of claiming the raffle prize.
Cardholders whose names are drawn will be subject to verification by Citi.
Winners will be notified via phone and registered mail.

All non-winning entries from previous Raffle Draws will be carried over in the succeeding Raffle Draws.

Winners shall be given sixty (60) calendar days from receipt of the notice by registered mail to claim their respective Prizes. Unclaimed Prizes will be forfeited by Citi with the prior approval of DTI.
Winners may claim their respective Prizes Mondays to Fridays (except holidays) from 9:00AM to 5:00PM at 16th Floor Citibank Square, #1 Eastwood Avenue, Eastwood City, Barangay Bagumbayan, Quezon City.

Upon claiming his/her Prize, the Winner must present the following:
Official Notification Letter from Citi;

Valid Card used for the winning transaction;
Two (2) valid forms of identification or government-issued I.D. (with photo and signature), including Driver’s License, SSS I.D., Passport, Birth Certificate, TIN card;
Valid Tax Identification Number (TIN) – Winners must submit their TIN number to claim their Prizes. Winners who do not have TIN numbers would need to secure a one-time TIN from the Bureau of Internal Revenue.
Upon claiming his/her Prize, the Winner must provide all the necessary information, make the necessary arrangements, obtain the required documents, and take all the needed action for him/her to be able to finalize plans for his/her Prize. The information needed includes but is not limited to the Winner’s choice of his/her Prize, the name and contact information of the Winner and his/her companions, the specific date when the Prize will be used (ex: date of travel, date of proposal event), and personal information needed to book flights and hotels and make other necessary arrangements.
The Winners must sign the Release, Waiver and Quit Claim form of Citi.

The following are additional conditions pertaining to the Prize:
The details of all Prizes, such as but not limited to the event dates, the suppliers, the airline tickets, the airline company, the flight schedules, the flight origin, the flight destination, the flight route, hotel accommodation, the room category, the hotel and the location of the hotel will be at the discretion of Citi.
Each Prize should be redeemed and fulfilled within the redemption period specified for that Prize. In the event that the Prize involves travel, the Prize should be completed within the specified travel period. If for any reason whatsoever, a Winner does not complete the Prize within the specified redemption period or travel period, the Prize will be forfeited by Citi with the prior approval of DTI.

In the event that the Prize involves travel, the Winner and his/her companions are responsible for (1) securing all travel documents including but not limited to, a current valid passport, visa, travel insurance or other travel documents acceptable to the airlines and destination country/ies, (2) passing all medical requirements, (3) respecting all local laws and regulations in the destination country/ies, and (4) ensuring full compliance with the regulations of all venues. Winners shall not have any claim against nor be entitled to any compensation from Citi in the event of their failure to secure the necessary travel documents (passport, visas, travel insurance etc.) and other travel requirements or to comply with all the requirements for travel to the destination country/ies.

Winners and their companions will shoulder all applicable airline/airport taxes (i.e. travel tax, terminal fee), government taxes, fees and surcharges and all other costs and expenses incurred in redeeming and using the Prizes.
Prizes do not include meals, transportation and pocket-money (other than those expressly provided for in the Prizes), excess baggage allowances, and all other additional costs and expenses. These shall be the responsibility of the Winners and their companions.
In the event that Citi is prevented from awarding the Prize or continuing with the Promo by any event beyond its control, including but not limited to fire, flood, epidemic, earthquake, explosion, labor dispute or strike, act of God or public enemy, satellite or equipment failure, riot or civil disturbance, terrorist threat or activity, war (declared or undeclared) or any federal, state or local government law, order, or regulation, public health crisis, order of any court or jurisdiction, or other cause not reasonably within Citi’s control (each a “Force Majeure” event or occurrence), then, Citi has the right to modify, suspend, or terminate the Promo.
Citi does not assume any responsibility for any Prizes or any products and services offered under the Promo. The Prizes are not certified by Citi and under no circumstances shall the inclusion of any Prize in the Promo be construed as an endorsement or recommendation of such Prize by Citi. The providers/suppliers of the Prizes are not agents, contractors, employees, servants, consultants or affiliates of Citi. All risks associated with the use of the Prizes shall be assumed by the Winners.
Each Winner should maintain his/her Card active and in good credit standing for a period of twenty-four (24) months from the end of the Promo Period or awarding of the Prize. Winners who qualified for raffle entries on account of enrolment for e-Statement or Citi Alerts, creation of a Citibank Online account, or application for a new supplementary Card/s should maintain his/her enrollment in the facility, his/her Citibank Online account, and/or the supplementary Card/s applied for, active for a period of twenty-four (24) months from the end of the Promo Period or awarding of the prize.
The 20% Prize tax (where applicable) will be shouldered by Citi.
The Prize is transferable but not convertible to cash, cash advance, or kind.
Each of the Winners authorizes Citi to disclose and publish his/her and his/her companion’s names, photos and other particulars to any person/s and in any mode or manner as Citi may deem appropriate. The Winners may be required to participate in publicity programs which Citi may from time to time organize in connection with the Promo and if so, such Winners must attend such functions. Expenses and/or costs in attending such functions will be shouldered by the Winner.
This offer cannot be used in conjunction with any other existing bank promotion.
In case of dispute regarding the Cardholder’s eligibility, coverage of dates, awarding, etc., Citi’s decision will prevail. All questions or disputes regarding the Cardholder’s eligibility for the Promo or for any request will be resolved by Citi.
Any fraud and/or abuse related to but not limited to the following instances, may result in the disqualification of the Cardholder from the Promo, the suspension or cancellation of his/her Card privileges, and/or the charging to the Cardholder’s account of the value of the Prize which ranges from Php 200,000.00 to Php450,000.00, at Citi’s discretion:
Credit card transaction;

Participation in the Promo;
Filing of the application/s, documents or requirements for the purpose of obtaining the Prize;
Cancellation of new cards e-Statement enrollment or new Citi Alerts enrollment, closure of the new Citibank Online account, and termination of the new supplementary card/s within a period of 24 months from the end of the Promo Period or from the awarding of the Prize; or
Cancellation of the Card within a period of 24 months from the end of the Promo Period or from the awarding of the Prize.
This shall be without prejudice to any legal action that may be taken by Citi.
